Modern Nutrition Practices Are Half Service, Half Digital Product

A typical RD private practice today blends one-on-one MNT sessions, package pricing, group programs, online courses or memberships, supplement affiliate income, and sometimes meal-plan IP licensing. Insurance reimbursement (now finally available for diabetes, CKD, and a growing list of conditions) sits alongside cash-pay sessions and digital revenue. Generic bookkeepers see deposits and book them all as one revenue line.

That works until tax planning starts. Digital course revenue, affiliate income, and live MNT services have very different QBI eligibility profiles, very different sales tax exposure across states, and very different cash recognition timing. Lumping them together hides which side of the practice is actually paying the bills and creates landmines at tax time.

We built our workflow around how RD practices actually operate. Every nutrition client gets revenue-stream segregation, deferred-revenue treatment for packages and programs, multi-state nexus tracking, HIPAA-aware financial workflows, and CPA oversight from PizzelloCPA, PLLC on the moves that move the tax bill.

What We Handle That Generic Bookkeepers Miss

Cash-Pay vs Insurance Mix Tracking

MNT insurance reimbursement has expanded but is still messy. We separate cash-pay sessions, insurance MNT, and package work in your chart of accounts so you can see which stream is funding the practice and which is breaking even after write-offs.

Package and Membership Plans Treated as Liabilities

Prepaid session packages, monthly memberships, and group program enrollments are unearned revenue until services are delivered. Booking them as cash income overstates revenue and creates phantom tax bills. We track them as deferred revenue and recognize as services are rendered.

Course and Digital Product Revenue Separation

Online course sales and evergreen membership revenue have different QBI eligibility, different sales tax implications by state, and different cash recognition timing than 1:1 MNT services. We segregate them in your chart of accounts so tax planning has clean inputs.

Affiliate and Supplement Income Tracking

Fullscript, Wellevate, and direct supplement affiliate income is real revenue with real tax consequences. We track it cleanly and flag any inventory or product responsibility implications when you carry stock vs drop-ship.

Multi-State Telehealth Nexus

Telehealth across the licensure compact still creates state income tax and sometimes sales tax exposure. We track revenue by state so we can surface nexus exposure before it becomes an audit.

HIPAA-Aware Workflows, BAA Available

For RDs touching PHI through insurance billing or EHR-integrated practices, we sign Business Associate Agreements and segregate financial data from clinical by design.

S-Corp Election and Reasonable Comp

MNT services are SSTB so QBI phases out fast, but digital product revenue may not be. The S-Corp election gets more interesting when revenue mix matters. We model your specific mix during onboarding to find the actual break-even.

Your Practice Software, Connected

Healthie, Practice Better, Nutrium, SimplePractice, Kalix. We pull billing data into QuickBooks Online cleanly so your clinical software stays the system of record for patient care, and QBO is the system of record for finance.

Common Questions

How do you handle prepaid package and membership plans?

As deferred revenue. We book the client payment as a liability and recognize revenue as services are delivered. The result is an accurate P&L and no phantom tax bill on money you have not earned yet.

Do you track online course and digital product revenue separately?

Yes. Course revenue, evergreen memberships, and 1:1 MNT services have different tax profiles, different state tax implications, and different cash timing. We separate them in your chart of accounts so tax planning has clean inputs and you actually know what's paying the bills.

How do you handle insurance MNT billing?

We reconcile EOBs against deposits, track contractual write-offs separately from revenue, and surface AR aging by payer. Insurance MNT now covers diabetes, CKD, and a growing list of conditions. The economics still need to be tracked separately from cash-pay.

Should my RD practice elect S-Corp status?

It depends on the revenue mix. For practices clearing about $80,000 in profit with mostly clinical revenue, yes. For practices with meaningful course or digital revenue, the math gets more interesting because not all revenue is SSTB. We model your specific mix during onboarding.

I sell supplements through Fullscript or Wellevate. How do you handle that?

We track affiliate or pass-through commission income separately from service revenue. If you carry physical inventory or drop-ship under your brand, we also flag the sales tax and product liability implications by state.

Do you offer a Business Associate Agreement?

Yes. RDs billing insurance or working in HIPAA-covered settings can request a BAA during onboarding. Financial data is segregated from clinical by design.

Do you integrate with Healthie or Practice Better?

Yes. We pull billing data from your practice platform into QuickBooks Online so your clinical software stays the system of record for client work, and QBO is the system of record for finance.

What does pricing look like for a solo RD?

Most solo practices start at our $199/mo Starter tier. Practices with online courses, group programs, or multi-state telehealth usually land in Grow ($349/mo) or Surge (custom). Every engagement includes CPA oversight from PizzelloCPA, PLLC.
