What moves the price for a therapy practice

Insurance is the biggest driver. Payers deposit net amounts that cover many sessions, so each EFT has to be tied back to the EHR payment report, and card payouts arrive net of processing fees. A cash-pay practice with one bank account skips most of that work.

Group practices add associate pay, supervisee splits, and payroll or contractor reconciliation. A practice with more than one owner adds per-owner profit allocations. Some providers do not serve that structure at all, which matters more than any price difference.

The last driver is what is bundled. A price that includes a tax return is not comparable to one that does not, and catch-up bookkeeping for past months is often priced separately. Add it all up for a year before choosing. If you are specifically weighing Heard, see Heard alternatives for therapists.

bookkeepz at a glance

bookkeepz, LLC is a remote bookkeeping firm for therapists, psychologists, and other clinical private practices, based in Greenville, North Carolina and serving clients nationwide.

Every engagement is led by Andrew Pizzello, CPA, and every client works with a dedicated bookkeeper inside the client’s own QuickBooks Online account.

Starter is from $199 per month and covers monthly transaction categorization, bank and credit card reconciliation, a monthly P&L and balance sheet, tax-ready books, and client portal access.

Grow is from $349 per month and adds enhanced financial reports, receivables and payables tracking, quarterly review meetings, a custom chart of accounts, and year-end tax prep support.

Surge is custom-priced and adds a licensed CPA’s review of every monthly deliverable by PizzelloCPA, PLLC, monthly review meetings, a live financial dashboard, cash flow forecasting, priority support, and multi-entity support.

CPA review is included only in Surge. Tax preparation and filing are a separate engagement with PizzelloCPA, PLLC, an affiliated licensed CPA firm, because bookkeepz is not a CPA firm and does not provide attest services.

Packages are billed monthly with no long-term contract and can be cancelled with 15 days’ notice. Mental health practices can request a Business Associate Agreement.

Common questions

How much does bookkeeping cost for a therapy practice?

At bookkeepz, Starter is from $199 per month and fits most solo therapists, Grow is from $349 per month for practices that need receivables tracking, a custom chart of accounts, and quarterly reviews, and Surge is custom-priced and adds CPA review of every monthly deliverable by PizzelloCPA, PLLC. Other providers’ published prices are in the table on this page, each as listed on their own site in September 2026.

What makes bookkeeping cost more for one practice than another?

The work, more than the size of the practice. Insurance billing adds EOB and deposit reconciliation, a group practice adds supervisee splits and associate pay, payroll adds its own reconciliation, and months of backlog add catch-up work. A solo cash-pay practice with one bank account is the simplest case.

Is tax preparation included in bookkeeping prices?

It depends on the provider, which is why monthly prices are hard to compare directly. Some bundle a return, some add it for a fee, and some quote it separately. At bookkeepz, tax preparation and filing are a separate engagement with PizzelloCPA, PLLC, our affiliated licensed CPA firm, quoted for your situation.

Do I pay extra to catch up months of old bookkeeping?

Often, so ask every provider how catch-up work is priced. Several providers in the table list their own catch-up pricing or promotions. At bookkeepz, a free 30-minute consult is where we look at how far behind your books are.
